All teachers and staff with direct student contact also have CPS (Child Performer Services) registration numbers and have been fingerprinted and processed accordingly.

All classes and workshops are run by First Dog Corp. dba John D’Aquino’s Young Actors Workshop (JDYAW). The purpose of the class or workshop is to provide aspiring actors and their families an education in acting and navigating the entertainment industry as it relates to child actors.

FIRST DOG CORP. DBA JOHN D’AQUINO’S YOUNG ACTORS INTENSIVE WORKSHOP (JDYAIW) IS A TALENT TRAINING SERVICE. THIS IS NOT A TALENT AGENCY CONTRACT. ONLY A TALENT AGENT
LICENSED PURSUANT TO SECTION 1700.5 OF THE LABOR CODE MAY ENGAGE IN THE OCCUPATION OF PROCURING, OFFERING, PROMISING, OR ATTEMPTING TO PROCURE EMPLOYMENT OR ENGAGEMENTS FOR AN ARTIST. THIS COMPANY IS PROHIBITED BY LAW
FROM OFFERING OR ATTEMPTING TO OBTAIN AUDITIONS OR EMPLOYMENT FOR YOU. IT MAY ONLY PROVIDE YOU WITH TRAINING AND COUNSELING. FOR MORE INFORMATION, CONSULT
CHAPTER 4.5 (COMMENCING WITH SECTION 1701) OF PART 6 OF DIVISION 2 OF THE LABOR CODE. A DISPUTE ARISING OUT OF THE PERFORMANCE OF THE CONTRACT BY THE TALENT SERVICE THAT IS NOT RESOLVED TO THE SATISFACTION OF THE ARTIST SHOULD BE REFERRED TO A LOCAL CONSUMER AFFAIRS DEPARTMENT OR LOCAL LAW ENFORCEMENT, AS
APPROPRIATE.

First Dog Corp and JDYAIW make absolutely no offer or promise whatsoever that you will receive representation or consideration for a job by a casting agent or other industry
professional. The purpose of our classes is to educate. There may be a performance of your craft in front of a live audience which includes industry professionals whose sole purpose is to evaluate your performance from their unique perspective. These professionals may include agents, managers and other industry professionals, but they are present to provide feedback and guidance only. After your event, you may receive a written or verbal evaluation of your performance based upon several characteristics and the comments and critique of the invited guests, your group leaders and JDYAIW staff.

To further comply with the law, First Dog Corp. DBA John D’Aquino’s Young Actors e Workshop (JDYAW) has acquired and has posted its required $50,000 surety bond and is fully
insured by Mount Vernon Fire Insurance Company as well (certificate available upon request). By agreeing to this contract you agree to allow us to use your photographic or videographic image on our website, flyers or for other promotional purposes only. All scripts and other intellectual property written or distributed by this company remain our sole property and all copyrights are enforced. You may not use any of this

material for any purpose other than for educational purposes without the express written consent of JDYAW.

YOUR RIGHT TO CANCEL

THE LAW GIVES YOU THE RIGHT TO CANCEL YOUR CONTRACT AND OBTAIN A FULL REFUND, WITHOUT ANY PENALTY OR OBLIGATION, IF NOTICE OF CANCELLATION IS GIVEN, IN WRITING,WITHIN 10 BUSINESS DAYS FROM THE ABOVE DATE OR THE DATE ON WHICH YOU COMMENCE

UTILIZING THE SERVICES UNDER THE CONTRACT, WHICHEVER IS LONGER. FOR PURPOSES OF THIS SECTION, BUSINESS DAYS ARE MONDAY THROUGH SATURDAY. TO CANCEL THIS CONTRACT, MAIL OR DELIVER OR SEND BY FACSIMILE TRANSMISSION A SIGNED AND DATED COPY OF THE FOLLOWING CANCELLATION NOTICE OR ANY OTHER WRITTEN NOTICE OF
CANCELLATION TO (NAME OF TALENT SERVICE) AT (ADDRESS OF ITS PLACE OF BUSINESS), FAX NUMBER (IF ANY), E-MAIL ADDRESS (IF ANY), AND INTERNET WEB SITE ADDRESS (IF ANY), NOT LATER THAN MIDNIGHT OF (DATE). IF THE CONTRACT WAS EXECUTED IN PART OR IN WHOLE
THROUGH THE INTERNET, YOU MAY CANCEL THE CONTRACT BY SENDING THE NOTIFICATIONTO: (E-MAIL ADDRESS). TO CANCEL, USE THE FOLLOWING CANCELLATION NOTICE: I YourName hereby cancel this contract. Dated: Date of Notice Artist Signature: Camper Name. If you cancel, all fees you have paid must be refunded to you within 10 business days after delivery of the cancellation notice to the talent service.
